The hold routing in the beginning is insane, and the late-spike bracket run is also insane. If it weren't for the mid-section potion, I think it could have been a 22 without much trouble.

You can just brush through the early hold brackets with one foot. For the late-game patterns, it's hard for locals to hit them perfectly, so use brackets/twists as much as possible to clear them.

VERY Cheatable, the 2 note timings near the end can be bracketed if you know what your doing without damaging your score. Other then that it's mostly brackets

It's an advanced 10. It is difficult, but it's not a 22.
